
A loaded barbell calf raise that alternates two directions: heels up into plantarflexion, then toes up into dorsiflexion. It is the only common calf variant that trains the tibialis anterior on the front of the shin as well as the calf behind it, which makes it a shin-splint drill as much as a calf builder.

Breathing & tempo — Exhale as you drive the heels up, inhale as you lower, exhale again as you pull the toes up. Two seconds in each direction with a one-second hold at both ends.
Where to start, and what to change when it stops being hard.
Start with the empty 20 kg bar even if you squat three times that. The front of the shin fatigues long before the calf does, and it is the honest limiter here. Add 5 kg only once you can hold a clean one-second pause at both ends for 15 reps. Most people never need more than 40-60 kg on this.
The errors that show up most often, and the cue that fixes each one.
Pause a full second at each end. Move at roughly two seconds per direction so the muscle, not momentum, changes your ankle angle.
Set a fixed soft knee bend before the first rep and treat it as locked. Film from the side once — if the knee angle changes, cut the load by a third.
Pull the toes up hard enough that you feel the front of the shin burn by rep eight. If you feel nothing there, you are rocking rather than contracting.

Almost every calf exercise trains one direction at the ankle. The rocking version adds the return trip: after the heel raise you pull your toes up off the floor against the weight of the bar and your own body, loading the tibialis anterior. That muscle decelerates your foot every time it lands when you walk or run, and nothing else in a typical leg day touches it.
Be honest about the calf half. You are standing on flat floor, so your heels stop at ground level and you never get the loaded stretch below neutral that a calf block or machine gives you. As a gastrocnemius builder this is a compromise. Its real argument is the toe-raise half, and the fact that a squat rack is the only kit you need.
At the end of a lower-body day, or as a standalone ankle session two or three times a week. If you run, cycle between this and a deficit calf raise so you get both the anterior work and a full stretch. Do not pair it with heavy squats in the same slot — a fatigued ankle under a loaded bar is a balance problem, not a training stimulus.

No. On flat floor your heels stop at ground level, so you lose the stretched portion of the range that drives most calf growth. What you gain is the tibialis anterior work on the way back. Use it as an ankle-balance drill, and use a calf block or a machine when calf size is the goal.
Because the tibialis anterior is far smaller and far less trained than your calves. That burn is the point of the movement and it is the normal experience for the first few weeks. It usually settles within four or five sessions as the muscle adapts.
It can. Medial shin pain in runners is often linked to a tibialis anterior that cannot handle the repeated eccentric load of foot strike. Strengthening dorsiflexion directly is one of the few interventions with a clear mechanism. It is not a cure for pain that is already present — back off mileage first, then build the ankle.
Yes, and for most people the barbell adds little. Standing on flat floor and rocking heel to toe with no load at all still fatigues the tibialis anterior within 20 reps. The bar mostly loads the calf half, which is the half this movement does worst.
